  • Medicare/Medicaid and /or private insurance cards must be available for us to copy and verify.
  • Name, address and contact numbers of the responsible party and one other person. Choice of doctor (medical, dentist, podiatrist, etc.)
  • Choice of pharmacy that follows facility and state guidelines.
  • Pay 30 days in advance for private pay and Medicaid pending.
  • Copies of DNR/Living Will.
  • Copies of POA/Guardianship.
  • All personal items must be marked, labeled or etched.
